For many families, the choice between staying at home and moving into a nursing facility feels forced. The NHTD waiver was designed to make a different option possible: the support and services that keep someone safely in the community.
The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver is a Home and Community Based Services (HCBS) program administered by the New York State Department of Health. The waiver uses Medicaid funding to provide the support people need to stay included in their community.
Participants come to the waiver one of two ways: transitioning home from a nursing facility, or choosing the waiver to avoid moving into one in the first place.
The waiver becomes an option when informal support and other Medicaid services aren’t enough to keep someone safely in the community. It can also apply when the waiver is simply a more efficient use of Medicaid funds.
Seniors and people with disabilities have the same rights as everyone else. They should be in control of their own lives, with the freedom to take on, manage, and learn from risks.
Every participant is paired with a Service Coordinator. Your coordinator helps you access services within the program and connects you with other local, state, and federally funded educational, vocational, social, medical, and other services.
Request the waiver program from the Regional Resource Development Center.
A nurse from the RRDC visits to evaluate whether you’re eligible.
Once you’re deemed eligible, choose a Service Coordination agency like Regency to assist with your Initial Service Plan (ISP) and get services started.
