The Social Care Network (SCN) program is a New York State initiative to help Medicaid members identify and access needed services and supports. These needs, once met, can lead to a major improvement in a person’s health and quality of life.
Regency proudly participates in the SCN program by providing screenings and navigation services. We are currently one of the largest SCN screeners in New York City.
